Spirituality and the Workplace Survey

Instructions:

For each statement in the following Survey, you are asked to select a response on a scale which ranges from 1 (Strongly Disagree) to 5 (Strongly Agree). If you are neutral (i.e., sometimes agree and sometimes disagree with a statement), or if you partly agree and partly disagree, then mark 3 (Neutral). If you feel you are not able to respond to a statement, please mark "Don't Know."

The Spirituality and the Workplace Survey is designed to help you understand how you can more effectively integrate humanistic principles and practices into your day to day functions. There are no right or wrong answers. Record your perceptions based upon your recent experiences with your organization.

The following is a list of definitions of important terms you will find in the Survey. Make sure you are clear about the meaning of the terms before responding to the Survey.

Workplace Spirituality:
An environment where ethics and values are an integrated part of core business functions.

Spirituality:
An individual's inner source of wisdom.

Ethics:
One's personal morality or a set of widely accepted moral principles.

Wisdom:
An in-depth understanding, empathy, and compassion for the human experience.

Leadership:
All those who make decisions and policies regarding the direction of the organization.

Empowerment:
The ability to perform your job with the least amount of oversight or management.
